What should I do if I need to cancel my appointment?
We request that you cancel your appointment 48 hours prior to your scheduled time to allow someone else to take your slot. In the event you do not cancel and fail to show up for an appointment, you will be charged your copay amount for the missed appointment.
Does your clinic offer telemedicine visits?
Yes we can do visits online for acute concerns or follow up appointments. Wellness visits and return to work visits must be done in person.
What should I do to prepare for my first appointment?
Be sure to arrive early to give yourself plenty of time to complete the initial intake process. You can also bring any relevant lab reports, medication, and treatment plans you’re currently under to discuss.
What happens if I don't show up for my appointment?

Does your clinic have evening hours or walk-in hours?
Walk-ins welcome during business hours. If you need an evening appointment please call ahead at (708) 581-4960 and we will try our best to accommodate your request.
Do I need a primary care provider (PCP)?
No matter how healthy you are, it’s a good idea to have a primary care doctor. Primary care providers act as the quarterback for all of your healthcare needs and are the first line of offensive when it comes to preventative medicine.
What ages do you see and treat?
We see patients ages one to 75.
What is evidence-based medicine?
“Evidence-based medicine” means medical care that is up-to-date with the most current recommendations. Medical research is constantly discovering new ways to identify, prevent, and treat illness. We pride ourselves on being aware of that research so we can provide our patients with the most effective medical care. We practice evidence-based medicine in our clinic
How do I get a prescription refilled?
To have a prescription refilled, please have your pharmacy call in a refill request to us at (708) 581-4960.
What are your COVID-19 guidelines?
We follow the CDC guidelines as it relates to COVID-19. We require face coverings, social distancing, and hand hygiene which all must be strictly observed while in our office.
What insurances do you take? Do you accept self-pay?

What kinds of payment do you accept?
If you have a deductible or co-pay, payment is due at time of service. Your insurance benefits will confirm the payment required. We accept cash, check, or credit card.
